What is the Forms module and how do I use it?

The Forms Module allows you to capture data from any participant in Enboarder. Forms are great at gathering information on new employees, collecting IT requests, or measuring sentiment and NPS! 📊

You can download them in Reports, or even have the results of a submitted Form sent automatically to a stakeholder in that journey! The possibilities are endless - and the data is game-changing. 💯

Adding a Form

To add your form, start by clicking the “Add Module” button within the Sequence where you’d like to add it, and then click Form.

You’ll see the Module Builder slide out from the right-hand side, featuring a Text Widget already populated. Don’t forget to add an Image Widget too! #Visual 💅🏼


Form Widgets

The Forms Module features the most Widgets of any Module, so let's take a look at them all below, but before we begin - as you're building your Form, ensure you give each Widget a unique Field Name so that you can pull them into dynamic tokens later on.

Side note: You can find the Field Name at the top of the Settings panel of any Form Widget. 😉

Text Field

The Text Field Widget is essentially our open-ended question field. This can be used for questions like "Describe a time when you learned something valuable about Team Play, and what was it that you learned?" or really any text question!

On the left, you can decide if it's a Required question, or if you'd like to Hide it.

  • Required means the receiver can't submit the form without answering that question

  • Hide means you could insert a Dynamic Token and answer that question for them, like entering their Managers name automatically for reporting purposes)

You can also decide between Short and Long Answers, essentially Long answers keep formatting for longer paragraph answers, and Short answers keep everything on one line.

Unique Field

The Unique field gives you the option of different field types to choose from:

  • Date

  • Time

  • Numbers

  • Email

  • Mobile

  • Profile

The Unique Field is similar to the Text Widget in the sense that the receiver can manually enter their answer. The difference between the two is that with a Unique Field you're able to control what they input.

For example, below you can see we're asking for their Email - so unless the user answers in the format something@company.com, Enboarder won't allow them to submit the form.

Dropdown

The Dropdown question is great for multiple options where the receiver can only pick one answer. It means that you could have multiple options but it conceals them within the question, as opposed to displaying them all (like the Multiple Choice does)

Multiple Choice

The Multiple Choice question allows you to provide a selection of answers displayed on the page to the receiver (unlike the Dropdown, which conceals all the possible answers in a menu).

It also gives you the option of a Single Answer or Multiple Answers, so the user can pick a selection.

Contact

The Contact channel question allows your participant to enter how they'd like to be communicated with - on the left in the settings panel, you can select the channels they will see - and if you're integrated with Slack or Teams, you can see that there too!

Note: If you are using this to allow them to update their preference, you will need to use an Update Value Module to ensure Enboarder updates their preference. Check that out here.

Rating

The Rating question is one of the most popular questions that you can use on a form. It allows the receiver to pick a numbered icon on a scale of your choosing, and then in your Reports and your Dashboard, you can even get averages of participants' answers!

In this example, we are using an NPS question (and yes, Enboarder does the calculation for you in Reports! ✅) which is a scale of 0-10. On the left in the Settings panel, you can pick from 1-3, 1-5, 1-6, and 0-10 (dedicated NPS score) - and then of course you can pick between the icons!

We recommend keeping 0-10 specifically for NPS.

File Upload

Need to gather some documents or even a selfie from your new hire? Using the File Upload, you can do just that! Allowing the receiver to upload a file up to 10MB, this handy question is used by lots of customers to gather and share a selfie of their new hire with the team.

They will be able to upload any of the following file types - Word Document, PowerPoint, Excel, CSV, PDG, ZIP, JSON, text/plain, rich text, PNG, GIF, JPEG, TIFF, RTF, or BMP.

Note: Enboarder keeps file uploads on the system for 30 days

Signature

The newest addition to the Form Widget family, the Signature Widget is used as a type of acknowledgment. When a participant gets to this question, it will prompt them to confirm their identity using a password method (after they receive a one-time pin).

Note: signatures don't store as a digital signature, instead, it's the signees name, date and time, and IP address that gets stored in Reports.


Form settings

The Form Settings will give you the following options:

  1. Forward the Form results as an email to stakeholders in the Workflow once the Form has been submitted.

  2. Forward the Form results as a webhook to another application in the Workflow once the Form has been submitted by the recipient.

  3. Send out the form as an anonymous survey

This is all to support customization and making sure that the Form results are going where they need to be! 📬

Forward Form Results via Email

This feature will allow you to send the Form results that have been submitted as an email to defined stakeholders in the Workflow. Defined stakeholders in the Workflow means that the stakeholder information has been added to the Workflow.

Forward Form results to a URL

The Forward Form results to a URL feature opens a world of possibilities in how you can share the Form results! ✨

This feature provides the power of webhooks so that you can send the Form results from Enboarder to another application!

Once this is ready, the Form settings should look like this:

Anonymous Surveys

Do you want to learn more about a topic while giving the responders anonymity? Anonymous Surveys are for you! ✅

Ticking the 'Turn form into an anonymous survey' box will do two things!

1. An anonymous form link - once the participant clicks on this link then they will be able to see the Form and answer it.

2. You get to choose which Brand the Form will display!

Note: The widget ‘File Upload’ and all Dynamic tokens other than ‘Brand’ do not work within anonymous survey links.
